  • Outreach
    The term outreach applies to a service delivery model that aims
    to improve accessibility of services to seniors. Outreach
    workers offer services to the client in the home, at the center
    or in the community as needed.

    Outreach is an effort to link people in need to appropriate
    community resources. Outreach takes the services of information
    and referral into the community. The goal of outreach is to identify
    persons in need and link them to available community resources
    because individuals are often unaware of services that exist in the
    community.

    Outreach Services core functions are: assessment, information
    and referral, completion of forms, supportive counseling, issue
    management, monitoring and following-up their continued well-being.
    Outreach workers engage in community development and provide
    education workshops in topics like sleep therapy, arthritis
    management, and incontinence.
